GAN SHELANU PRESCHOOL 2021-2022 SCHOOL YEAR REGISTRATION FORM

In abidance with Gan Shelanu's COVID Procedures and Protocols, a limited number of spots will be available in an effort to keep class sizes to a minimum and cohorts together.  Priority will be given to students currently enrolled in Camp Mishpacha and children who were enrolled in Gan Shelanu for the 2020-2021 school year.  These children will have priority if all their registration materials and deposit are received by Friday, March 26.  Please understand that we will strive to meet the wants of all families but we will process all applications according to the number of spaces available.

 

In an effort to keep class sizes to a minimum and cohorts together, at the present time the Gan is only accepting a limited number of infants and transitional toddlers on a full time, 5 day/week schedule. Part time options may open in the future. 

 

Gan Shelanu Preschool will continue to follow guidance set by the Minnesota Department of Health, the CDC and the synagogue's COVID committee and will continue to monitor all communication from these sources. The health and safety of our students, their families and our staff continue to remain paramount. 

 

 

ENROLLMENT OPTIONS:

 

Infant  (Babies 26 weeks to 16 months as of September 1, 2021)

Transitional Toddler  (Children beginning at 16 months as of September 1, 2021)

Toddler  (Children 20-33 months as of September 1, 2021)

  • 5 full days (Monday-Friday, 7:30 am-5 pm)  $1,450/month
  • 5 school only days (Monday-Friday, 8:45 am-12:45 pm)  $625/month
  • 3 school only days (Monday, Wednesday, Friday, 8:45 am-12:45 pm) $500/month

Preschool  (Children 34-48 months as of September 1, 2021)

  • 5 full days (Monday-Friday, 7:30 am-5 pm)  $1,365/month
  • 5 school only days (Monday-Friday, 8:45 am-12:45 pm)  $567/month
  • 3 school only days (Monday, Wednesday, Friday, 8:45 am-12:45 pm) $452/month

PreKindergarten (Children 49 months and older as of September 1, 2021)

  • 5 full days (Monday-Friday, 7:30 am-5 pm)  $1,365/month
  • 5 school only days (Monday-Friday, 8:45 am-12:45 pm)  $567/month

REGISTRATION DEPOSIT:

A non-refundable $225 deposit is due at the time of registration. 

 

Please note: This online registration form requires credit card payment of non-refundable deposit with a 2.5% credit card processing fee. If you wish to pay the deposit by check, please email janices@adath.net to receive a paper form and mail it to Adath with your check made out to "Adath Jeshurun Congregation".

First Emergency Contact

In the case of an emergency or injury requiring medical attention, please indicate two (2) people who you authorize to assume temporary responsibility for your child if a parent/guardian cannot be reached. The people MUST be able to drive and be willing to come within 30 minutes if called. All requested information must be completed in full.

Please note: If your child has asthma and/or severe allergies that may or may not require an EpiPen or other medications, your child's physician must complete an Emergency Care Plan prior to your child beginning Gan Shelanu. This form needs to be completed by your child's physician and submitted to the Gan Office one month prior to your child's enrollment date. No child who requires an Emergency Care Plan for asthma and/or severe allergies will be allowed to begin Gan Shelanu if this form and all medications (if needed) have not been submitted to the Gan Director. Please also submit a current photo of your child when you turn in your child's Emergency Care Plan.

During the course of the year, your child may participate in special programs to which we would like to invite their grandparents and/or the "special people" in your child's life. Please provide us with the name and complete address of grandparents and/or "special people" below even if they do not live in the Twin Cities area.

Payment Options and Authorizations

Please note that if your child is a current Gan Shelanu Preschool student, the payment option you have already chosen will be in effect the entire time your child is attending Gan Shelanu Preschool. Permission to Use Pictures and Images
I give permission for Adath Jeshurun Congregation to use pictures or images of my minor child named in this application in Adath marketing materials.

Gan Shelanu Preschool Admission Policy

When you register your child for Gan Shelanu, please be aware that we are committed to holding a space for your child in the age-appropriate class. Remember, if you withdraw your child, we may not be able to fill that space. Unfilled spaces in classes that have been budgeted, cost the education program thousands of dollars it cannot afford to lose. Please be mindful of this when you register and remember that your child is committed to attending the program you are registering for today.

1. The signed registration form must be accompanied by a $225 non-refundable registration fee.

2. I understand that in order for my child to be registered for the 2021 - 2022 Gan Shelanu Preschool school year, I must complete and submit the Registration Form, Preschool Admission Policy, Payment Authorization form (for new students) and a $225 non-refundable registration fee.

3. NO refunds are made for absences due to illness, bad weather, vacations or holidays. This policy is in accordance with other preschools and child care centers throughout the Twin Cities. Absences CANNOT be made up on other days.

4. It is agreed that your child is enrolled for the entire school year. All changes to your child’s schedule require a 30-day written notice.

5. It is agreed that your child’s Gan Shelanu tuition and all child care fees shall be paid using one of the options on the Payment Enrollment Authorization Form.

6. If a child is unable to adapt to the program, requiring more care than can be reasonably given in a group setting and exerting a negative impact on the other children in the group, the Director will meet with the parent/guardian to discuss the concern, and create a plan of action. If the situation does not improve within the mutually agreed upon timeframe, Gan Shelanu reserves the right to exclude the child. Gan Shelanu’s first obligation must be to maintain a healthy environment for all the children in its care.

7. Any change in a child’s schedule must be made in writing and requires a 30-day written notice. Changes will be made on a space availability basis and you will be charged a $50.00 change fee.

8. Gan Shelanu has a responsibility, rooted in Jewish tradition, to protect the health and safety of children and staff attending the school. It is understood that Gan Shelanu does not recognize the conscientious exemption and will not enroll a child or allow a child to remain enrolled unless he or she qualifies for the medical exemption or is vaccinated against all the diseases listed on the school’s Immunization Form, even if vaccination is contrary to the conscientiously held beliefs of the child’s parent or guardian.

9. All withdrawals from Gan Shelanu Preschool Program require a written notice 30 days prior to your child’s withdrawal date.

10. If you withdraw your child from Gan Shelanu Preschool Program at any time and at a later date wish to re-enroll your child, you will need to register your child as a new student. All admission policies will apply.

11. The Adath Jeshurun Congregation bills by the calendar month.

Gan Shelanu Beit Tinokot (Infant Room) Admission Policy

1. Registrations will be accepted on a first come first serve basis in the Beit Tinokot Program according to this policy: Adath members may register and hold a spot six (6) months before their child will begin the Beit Tinokot Program. Non-members may reserve a spot up to three (3) months in advance before their child will begin the Beit Tinokot Program. During the time of the COVID-19 pandemic, Gan Shelanu will strive to maintain this policy knowing a limited number of spaces will be available.

2. The completed registration form for Beit Tinokot must be accompanied by a $250 non-refundable registration fee to Adath Jeshurun Congregation.

3. Gan Shelanu will prepare for your child’s entrance according to the start date that you designate. If you delay your start date, you must pay for the months extended or forfeit your spot.
